Anthem conducted an off-cycle review and updated their Implantable Peripheral Nerve Stimulation Device Policy as a Treatment for Pain (SURG.00158). They added a positive coverage policy stating that an implantable peripheral nerve stimulator device (ReActiv8) is medically necessary for the treatment of chronic low back pain when all the detailed criteria have been met.
Other implantable peripheral nerve stimulation devices are still considered investigational and not medically necessary for individuals who do not meet the above criteria. This policy was published on April 16, 2025.
